•Rehabilitation Services:
At Grace Healthcare, exceptional therapy services begin with a clear understanding of each resident's goals.
The highly skilled rehabilitation experts at Grace Healthcare are committed to returning the highest levels of function and independence in a specialized rehabilitation environment. These professionals work closely with the physician and entire care team to coordinate an individualized therapy program specific to the needs and goals of each resident.
Grace Healthcare boasts on-site Physical Therapy, Occupational Therapy, and Speech-Language Pathology programs to deal directly with each area of functional loss. Our clinicians are licensed professionals with the education, experience and advanced expertise necessary for successful rehabilitation outcomes.

•Skilled Nursing Services:
These services are often provided after a hospital stay of at least 3 days and are usually covered by Medicare Part A, if the resident is otherwise qualified for this program. An interdisciplinary care team consisting of the attending physician, registered nurse, social worker, activity professional, dietician/nutritionist, consults with the resident and family to develop an individualized medical plan of care. This care plan is assessed and modified frequently to enable each resident to achieve his/her optimum level of care; physically, socially and emotionally.
Other skilled services provided may include post surgical care; stroke recovery; IV therapy services; respiratory therapy; oxygen and other medical gas administration; wound management; tracheostomy or illeostomy care; and tube feeding services. After completing these shorter term, high level treatment programs, most residents are able to returrn to their home or to a less intensive healthcare setting.
•Intermediate Care Services:
These services are provided to those who require daily monitoring and management of a condition that cannot be provided in a home setting. These services include medication management, licensed nurse supervision, assistance with bathing, dressing, and eating and catheter or ostomy care. These residents also benefit from the individualized, professional care planning process. Daily nursing restorative care may include incontinence care, bowel and bladder training, and ambulation training and assistance.
•Respite Care:
Respite Care is a short-term program designed to give caregivers a needed break from the extreme demands of caring for the chronically ill or handicapped person at home. If for a few days or a few weeks, each respite care resident receives all of the services normally provided in the home, in a secure medical environment.
•Alzheimer’s Care:
Many of our nursing centers have dedicated units for the treatment of Alzheimer’s disease and related disorders. These units are secure, safe environments especially focused to treat individuals with cognitive disorders. Special activities and programs are administered by staff who have been specially trained in the care of those who are cognitively impaired.
•Assisted Living Services:
Some Grace Healthcare facilities offer this level of service for individuals needing less intensive services provided in a safe, supportive residential environment. Grace provides state licensed assisted living services which include: housekeeping service, three meals daily with snacks, laundry service, general monitoring for health and well being, assistance with medication management, assistance with dressing, bathing, eating, social activities, assistance with scheduling medical appointments, and 24-hour supervision by trained healthcare providers.
•Adult Day Care Services:
Some Grace Healthcare facilities provide adult day care services. This offers a protective environment to those who may be living with family members but are unable to remain alone during the day.
These services include: two meals with an afternoon snack (special diets accommodated), social activities, beauty/barber shop services, supervision with basic health monitoring, and emergency nursing care. Services are generally provided from 7:00 A.M until 6:00 P.M. Monday through Friday. These services are offered currently at the Leewood Healthcare Center location in Annandale, VA (ph. 703-256-9770).
•Independent Living Centers:
Live life without the major concerns of home ownership such as yard work, building upkeep, appliance repair, or countless others. Experience the peace of mind that results from knowing emergency nursing response is available 24 hours a day. Independent living centers enable you to socialize with friends and family in a safe, secure environment. Grace Healthcare offers an independent lifestyle with amenities such as: social and recreational programs, transportation services, housekeeping services, security monitoring, restaurant-style dining, and postal services all on a campus that includes assisted living and skilled nursing services. These services are offered currently at Renaissance Marquis Retirement Village located in Bella Vista, AR (ph. 706-295-0014).
